I was thinking a lot about the moment in my last class. After the gist listening, the question was "Who is he singing to?" and Sara answered "Um... John Lennon?" Clearly, there are things I could have done to check the understanding of the questions.
But it drove home a point that you had made during feedback to our whole class. That QUESTIONS are very difficult for ESL learners. I sort of accepted what you were saying at the time, and I can see how it is true. Now I get it much more strongly.
